Everything has been pre-approved, there are no additional fees. The certificate is included in the back of the book. Medical Nutrition and Disease (Activity# 138395) is a self-study (720) print program and can be found on the CDR Web site by searching by provider (HARK), Program (Medical Nutrition and Disease), or Activity# (138395). This was an excellent review of nutrition and for me PERFECT! I am currently not practicing and wanted to see what I could still remember and sharpen my skills. UNFORTUNATELY I CAN NOT PRINT THE CERTIFICATE OF COMPLETION. It was said here that it was valid until August 2020 however when I completed everything and went to print the certificate it wouldn't print and was dated as not valid after 2019! It was not a lot of money but a lot of time that now I will get no credits for!
